HOW TO ENROLL:

(For New Students and Transferees)

  1. Place your documentary requirements inside long brown envelopes (1 envelopes each for Registrar, Guindance, and Clinic), labeled with Full Name, Course, and Contact Number and Office. (ex. Registrar's Office)
  2. Show your documentary requirements to the Registrar's Office. Then accomplished the enrolment form. You may get a copy of the form through this link: Enrolment Form
  3. Submit the medical requirements and Pre-enrolment Health Form to the Clinic. (Note: Print your consultation and pre-enrolment forms using A4 size paper upon enrolment.)
  4. Accomplish and submit Individual Inventory Form (front and back using black ballpen) to the Guidance Office, together with 2x2 picture with a nametag. You may download the form through this link: Enrolment Form
  5. Go to dean's office for advising and evaluation.
  6. Visit the MIS Office for the release of ISMIS username and password.
  7. Submit the accomplished enrolment form in the Registrar's Office and wait for the registrar personnel to check the completeness of your submitted requirements.

(For Old Students and Returnees)

  1. Accomplish and submit clearance to the Registrar's Office.
  2. Make sure to have complete grades.

(For Irregular Students)

  1. Visit your dean or chairperson for evaluation.


ENROLMENT FLOW

  1. Get a Shiftee form and submit the accomplished form to the Registrar's Office.

(For All Students)

  1. Proceed with online enrolment in ISMIS
  2. Wait for the approval of the enrolment
